How to Become a Nivano Physicians Member - Your Healthcare Journey

Choosing Nivano Physicians gives you access to quality care throughout Northern California. We work with multiple managed care plans and provide comprehensive healthcare services within California’s budget framework.

Step 1: Enroll in a Contracted Health Plan
To become a Nivano member, you must first join one of our contracted plans—Aetna, Anthem Blue Cross, or Molina Healthcare.

If you’re not enrolled, contact Health Care Options at 1-800-430-4263 (TTY: 1-800-430-7077) or visit healthcareoptions.dhcs.ca.gov to switch plans.

Step 2: Select a Primary Care Provider (PCP)
Once enrolled, choose a PCP within Nivano’s network:

Adults: Internal Medicine or Family/General Practice physicians
Children: Family/General Practice or Pediatrics physicians

You can change your PCP anytime by calling your plan’s customer service: Anthem 866-755-2680, Aetna 855-772-9076, Molina 800-898-9892.
