Nivano Physicians works with Sacramento County to support the Healthy Partners Program. We provide primary and preventive healthcare to low-income, undocumented adults in Sacramento County.

Eligibility Requirements for Sacramento County Residents

To qualify for the Sacramento Healthy Partners Program, you must meet the requirements set by Sacramento County health officials.

Who can join:

You must be 19 or older and live in Sacramento County. Your income must be at or below 138% of the Federal Poverty Level.

You must also have one of these:

  • Restricted scope Medi-Cal with an applicable Aid Code, OR
  • No health insurance and cannot get health insurance through normal ways

These requirements help the program serve those who need healthcare services the most. Our team works to help you sign up for the program.

Who is eligible for the Sacramento Healthy Partners Program?

The program covers primary care services. This includes health screenings, preventive care, immunizations, chronic disease management, women’s health services, behavioral health support, laboratory tests, and radiology services. You can get prescription medications at low cost through participating pharmacies.

Primary care services, health screenings, and laboratory tests are free for people who qualify. You pay for prescription medications, but they cost less at participating pharmacies.

Member Services is available Monday through Friday. Staff will check if you qualify and help you sign up. They will set up your first medical appointment. For more help, contact Nivano Physicians for assistance.

Apply through the Sacramento County Department of Human Assistance. You need:

  • Photo ID
  • Proof you live in Sacramento County
  • Income records

 

Your family income must be at or below 138% of the federal poverty level to qualify.

The program has physicians, nurse practitioners, behavioral health specialists, and other medical professionals. They focus on primary care, preventive medicine, and chronic disease management. All providers give evidence-based healthcare services.

Prescription medications are not free. But they cost much less at participating pharmacies. The program helps you find low-cost medication options so you can get the treatments you need.

The program mainly covers primary care and preventive services. But your doctor can help you see a specialist when medically necessary. They will work within program guidelines to make sure you get care for complex health conditions.
