Your Right to Opt Out
Under the California Consumer Privacy Act (CCPA) and the California Privacy Rights Act (CPRA), California residents have the right to direct a business to stop:
- Selling their personal information to third parties
- Sharing their personal information with third parties for cross-context behavioral advertising (such as targeted advertising based on activity across different websites or apps)
Nivano Physicians does not sell your personal information for money. However, we may share certain information — such as website analytics data and cookies — with third-party service providers in ways that may qualify as “sharing” under California law.
You can opt out of this sharing using the form or methods below.
What Information May Be Shared
The categories of personal information that may be shared with third parties for cross-context behavioral advertising purposes include:
- Identifiers: IP address, cookie identifiers, device identifiers
- Usage Data: Pages visited, links clicked, time spent on pages
- Inferred Data: General geographic location inferred from IP address
This data is collected through tools including Google Analytics 4. It is used to analyze traffic and, where applicable, to enable interest-based advertising. No medical records, PHI, or HIPAA-protected information is included.
How to Opt Out
Option 1 — Submit an Opt-Out Request Online
Complete the form below to submit your opt-out request. We will process your request within 15 business days.

To submit your request online, email compliance@nivanophysicians.com with the subject line “CCPA Opt-Out Request” and include the following information:
- Your first and last name
- Your email address
- Whether you are a website visitor, newsletter subscriber, or other
- A statement that you request Nivano Physicians stop sharing your personal information for cross-context behavioral advertising
Web team implementation note: Replace this section with a WordPress contact form (e.g., Contact Form 7, WPForms, or Gravity Forms) collecting: First Name, Last Name, Email Address, relationship type (visitor/subscriber/other), and an opt-out checkbox. Form submissions should route to compliance@nivanophysicians.com. Until the form is live, the email method above satisfies the CCPA requirement.

Option 3 — Browser-Level Opt-Out
You may also opt out of certain data collection at the browser or device level:
- Google Analytics Opt-Out: Install the Google Analytics Opt-Out Browser Add-On
- Global Privacy Control (GPC): Enable the GPC signal in a supported browser (e.g., Firefox, Brave, DuckDuckGo). We honor GPC signals as opt-out requests under CPRA.
- Cookie Settings: Manage cookie preferences through your browser settings
Your Additional California Privacy Rights
In addition to the right to opt out of selling or sharing, California residents also have the right to:
- Know what personal information we have collected about you
- Delete personal information we hold about you (subject to exceptions)
- Correct inaccurate personal information
- Limit use of sensitive personal information
- Non-Discrimination — We will not treat you differently for exercising your rights

How We Process Your Request
Once we receive your opt-out request:
- We will acknowledge receipt within 10 business days
- We will process your opt-out within 15 business days
- We will notify third-party service providers of your opt-out
- Your opt-out preference will remain in effect for 12 months before we may ask you to re-authorize sharing
We will not ask you to create an account to submit this request. We will not deny you services or charge different prices because you exercised your opt-out right.
Authorized Agents
A California resident may use an authorized agent to submit an opt-out request on their behalf. If you are submitting a request as an authorized agent, please include written authorization from the California resident along with your request.
