IMPLEMENTATION NOTES FOR WEB TEAM

This file contains two components:

  1. The TPMO Disclaimer Block — A short, CMS-required statement that must appear on every Medicare marketing page, in every marketing email to beneficiaries, and on all print/digital marketing materials that meet the CMS definition of “marketing.” This must be placed prominently — not buried in a footer — on the /medicare/ page, /medicare-annual-enrollment-period/ page, and any other page discussing Medicare Advantage plan benefits, costs, or enrollment.
  2. The Full Disclosure Page — A complete standalone page at a URL such as /medicare-plan-disclosure/ that provides the full TPMO context, language assistance information, and contact resources. The short disclaimer block on marketing pages should link to this page.

CRITICAL: Before publishing, replace 10 with the actual count of individual Medicare Advantage and Part D plans (not organizations) that Nivano Physicians represents in each beneficiary’s service area. The organization count is 3 (Aetna, Alignment Health Plan, Imperial Health Plan) but the plan count varies by county and must be calculated per service area.

About Our Medicare Plan Representation

Nivano Physicians works with licensed insurance agents to help Medicare-eligible individuals in Northern California understand their Medicare Advantage coverage options within our physician network.

We do not offer every Medicare Advantage or Part D plan available in your area.

Currently, Nivano Physicians represents the following 3 organizations:

  1. Aetna
  2. Alignment Health Plan
  3. Imperial Health Plan of California

These organizations offer a combined total of 10 Medicare Advantage products in our service area counties (Alameda, Butte, Contra Costa, El Dorado, Nevada, Placer, Sacramento, Santa Clara, Sutter, Yolo, and Yuba Counties).

Not all plans are available in every county. Plan availability, benefits, costs, and networks vary by location.

CMS Material Approval

Marketing materials that reference specific Medicare Advantage plan benefits (dental, vision, fitness programs, premium amounts, etc.) must be reviewed and approved by the applicable Medicare Advantage carrier prior to publication and, where required, filed with CMS through the Health Plan Management System (HPMS).

All plan-specific benefit claims on this website have been reviewed and approved by Aetna Medicare, Alignment Health Plan, and Imperial Health Plan of California prior to publication.

Web team note: This carrier approval statement should appear only on pages describing specific plan benefits. Pages with general eligibility information only do not require it. Confirm approval status with each carrier compliance contact for each applicable page.


Annual Update Requirement

CMS updates Medicare Communications and Marketing Guidelines (MCMG) annually. TPMO disclaimer language, plan and organization counts, and Language Assistance Notice requirements are updated each contract year, effective September 30.
